That is pretty much all I used it for ... can get photocopies and even better forms are out there.
Get Hancock instead:
Try "The Practitioner's Pocket Pal, Ultra Rapid Medical Reference" 2nd ed. by Jim Hancock (about $17).. Maxwell's is very cute but very small.
NOT MUCH HELP!!:
THIS WAS NOT AT ALL WHAT I EXPECTED. LIMITED INFO. VERY, VERY SMALL PRINT. I WOULD NOT BUY ANOTHER. FLIMSY.
